Acceptance

Rules for acceptance and participation in the program are the same for everyone without regard to race, color, age, sex, national origin, disability, political belief, or religion.

Cancellation, Withdrawal & Dismissals 

​

House of Faith-Abilene (hereinafter referred to as HOFA) reserves the right to dismiss any camper whose influence or conduct becomes detrimental to the best interests of the overall program. No refund from camp fees will be made for absences, withdrawals or dismissals before the end of the camp session and it will be the camper's parent or guardian's responsibility for picking up the camper in the event they are sent home.

Insurance

In the case of sickness, injury or an emergency, HOFA requests insurance information be on file. Should a camper require special medical treatment, prescription medicine or hospital care during the camp session, the medical claim will be filed on the camper's insurance and expenses will be forwarded to the parents or guardians

Transportation

HOFA has permission to transport my child to and from camp locations

​

Medical Information

​

In the event of sickness, injury or an emergency, I hereby give my permission to the physician selected by the HOFA sponsor to hospitalize and secure proper treatment (including anesthesia) for my child, and I do hereby agree to indemnify and hold harmless HOFA and any HOFA representative from any claim by any person whomsoever on account of care and treatment of said participant.

Photos and Video Footage

​

I understand the HOFA takes photographs and video/audio footage of children and/or adults involved in the HOFA ministry. I consent to the use of any such audio or visual record of the camper to be used, distributed, or displayed as agents of HOFA see fit.
